Cancellation Policy and Process
If you wish to cancel your services within 60 calendar days of enrollment, please fill out the following form: 60 day – Cancellation Form. Then fax it to (800) 514-7450 or (800) 931-8358. You will then be contacted within 24–48 hours by a senior consultant to process your refund. In order to qualify for a refund upon cancellation, this document must be received by fax, email or mail within 60 days of enrollment.
